I337M Reboot Loop Most Likely Caused by SD Card

My Galaxy S4 I337M was picked up from a Telus store, brand new in October 2014. Came with Kit Kat, which I rooted not too long after. I updated it to Lollipop and rooted in June of this year (5.0.1 OC4). I've had my SanDisk Ultra 64GB in the phone and using it actively (Camera auto-saves to the SD and my music is played from files on it) since day 1 without issues, until last week.

No apps are stored on the card. I don't have nor play any games on the phone.

Last week Thursday (Aug. 20) at night while just reading news via Twitter, the phone suddenly shut off and rebooted itself. Not 10 seconds after booting back up it rebooted again on its own. Repeat for another 5 times before I decide to take the battery out to stop it. The phone felt quite hot.

I left the card and battery out of the phone over-night, hoping it will resolve whatever issue the phone was having.

Morning time (ard 6am) came and I put everything back in and booted up. Seems fine. Went about my day including taking some photos (went out of town with wife and son). Around 5pm when I checked the photos via Gallery app, I noticed the photos didn't save properly (have the corrupted image in its place). Checked my auto-backup Dropbox and OneDrive storage and either didn't have the images or they were blank images as well. Fuuu!!!

Continued using the phone for a bit at that point only to have it start reboot-loop again. Took the SD out and continued using the phone all weekend. No reboot.

Searched around Google as well as XDA with no solid resolution. I had ordered a new Samsung EVO 64GB card on Friday which just arrived now (Amazon Prime FTW) and it's in my phone; so far so good (took a photo and copied a bunch of music files to it already). Both cards I didn't do any formatting initially; just popped them in and began using.

One of the threads I found seemed to have a resolve but my "always scan" option is already unchecked:

Will reply here with updates (knock on wood no more issues).
